POPI ACT

Eyetek Optometrist as a registered medical practice is required to obtain collect, process and retain Personal Information from our patients to enable us to render services to the patient and to obtain payment for the said services. 

Eyetek Optometrist undertakes to at all times comply with both the Health Professions Act and the Protection of Personal Information Act, especially with regard to the handling of our patients’ personal information.


What does it entail?

Personal Information is defined in the Protection of Personal Information Act and includes, among other things:

  1. Information relating to the race, gender, sex, pregnancy, marital status, national, ethnic or social origin, colour, sexual orientation, age, physical or mental health, well-being, disability, religion, conscience, belief, culture, language and birth of the person;

  2. Information relating to the education or the medical, financial, criminal or employment history of the person;

  3. Any identifying number, symbol, email address, physical address, telephone number, location information, online identifier or other particular assignment to the person;

  4. The biometric information of the person;

  5. The personal opinions, views or preferences of the person;

  6. Correspondence sent by the person that is implicitly or explicitly of a private or confidential nature or further correspondence that would reveal the contents of the original correspondence;

  7. The views or opinions of another individual about the person; and

  8. The name of the person if it appears with other personal information relating to the person or if the disclosure of the name itself would reveal information about the person.


WHAT THE POPI ACT MEANS FOR OUR PATIENTS

When you become a patient of Eyetek Optometrist you are requested to complete documentation requesting certain personal information that we require about our patients, during the course of treatment and services we will gather further information that will become part of your medical record and furthermore communication takes place between the practice and the patients relating to the findings and results of treatment as well as promotional material.

We only collect, process and retain personal information about our patients that we legally require and which serves a purpose and we will never request any personal information that we do not require.

All our patients’ records are stored as per the Health Professions Act and are destroyed at such dates as we are required to do by law.
